Exactech Pte Ltd, a company that provides surgeons with orthopaedic implants, called for their polyethylene inserts to be removed from circulation, as they do not last as long as expected, which can lead to serious problems for some patients. Many of these knee replacement parts have been on the US market for almost 30 years.
Exactech Ultra-High Molecular Weight Polyethylene (UHMWPE) knee inserts were intended to last for eight years. However, reports from customers who have received knee replacements using Exactech’s polyethylene knee inserts found the the inserts to be faulty and vulnerable to premature failure, leading to heightened risk of additional injury to your joints and other health complications.
The lifespan of a knee replacement often comes down to the durability of their polyethylene insert. Typically, inserts are designed, manufactured and packaged to be resistant to oxidation before being inserted into your knee, which is a prominent cause of premature failure. Exactech’s error was not including a layer of Ethylene Vinyl Alcohol (EVOH) in the packaging, even though their designs implied it would be there. The absence of that layer increases the chances that oxygen can leek into the insert and cause degeneration. That degeneration can cause further injuries to the patient.
Exactech’s failure to include Ethylene Vinyl Alcohol in the packaging also applies to their ankle inserts. After several doctors and patients complained of the premature failures the FDA to issue a Class 2 Device Recall. If you were given a defective ankle replacement, you may need revision surgery, in addition to experiencing significant pain, not to mention the extra medical bills and time spent off work.
There are four components of a hip replacement:
A couple of studies that came out in 2020, one titled Early Polyethylene Failure in a Modern Total Hip Prosthesis: A Note of Caution and another titled Early failure of a modern moderately cross-linked polyethylene acetabular liner, both noted that the acetabular liner in Extach’s hip replacements were lasting several years fewer than expected.
Exactech’s Connexion GXL acetabular liners were recalled because of the “risk of edge-loading and premature prosthesis wear,” meaning they could wear down easier than expected when the femoral head puts pressure on the edge of the hip socket. The polyethylene acetabular hip liners are prone to the same oxidation as the knee and ankle inserts both before and after implantation. Before you are inserted with the replacement, the same packaging defects as the polyethylene inserts allow more oxygen to seep through. After surgery, they can become oxidized if the following risk factors are true:
The recall applies to the Exactech hip replacement components listed below:
