Exactech Pte Ltd, a corporation that provides surgeons with orthopaedic implants, issued a global recall in February 2022 for their polyethylene inserts and hip liners, as a mistake in packaging resulted in defects. Many of these joint replacements have been implanted in patients for almost 30 years.
The the insert in question, Exactech Ultra-High Molecular Weight Polyethylene (UHMWPE) knee inserts, were said to have an eight-year shelf life. However, complaints from patients who were implanted with them found the product to be flawed and susceptible to rapid degeneration, leading to pain, stiffness, fractures or even bone loss.
The lifespan of a knee replacement often comes down to the longevity of their polyethylene insert. Typically, inserts are designed, built and packaged to not oxidize before your surgery, which is a major cause of premature failure. Exactech designed and manufactured their inserts well, and intended to include a layer of Ethylene Vinyl Alcohol (EVOH) in the packaging to eliminate the risk of oxidation. However, it was discovered that, for whatever reason, EVOH was not included in the packaging, leaving the inserts prone to oxidation and degeneration. The knee replacement failing earlier than expected can cause significant harm to the patient.
Exactech’s failure to include Ethylene Vinyl Alcohol in the packaging also applies to their ankle inserts. After numerous surgeons and ankle replacement recipients reported the premature failures the FDA to institute a Class 2 Device Recall. A couple of academic studies published in 2020, one titled Early Polyethylene Failure in a Modern Total Hip Prosthesis: A Note of Caution and another titled Early failure of a modern moderately cross-linked polyethylene acetabular liner, both warned consumers that the acetabular liner in Extach’s hip replacements were lasting several years fewer than expected.
The recall was issued because Exactech’s Connexion GXL acetabular liners carried the “risk of edge-loading and premature prosthesis wear,” meaning they could wear down easier than expected when the femoral head puts pressure on the acetabular metal shell. As opposed to Exactech’s polyethylene ankle and knee liners, these hip liners are also prone to oxidation after they are inserted.
