Exactech Pte Ltd, a corporation that manufactures orthopaedic implants, called for their polyethylene inserts to be taken off the market, as they were found to fail early. Many of these joint replacements have been implanted in patients for almost 30 years.
Exactech Ultra-High Molecular Weight Polyethylene (UHMWPE) knee inserts were intended to last for eight years. However, in several circumstances the product did not last anywhere near that long, leading to pain, stiffness, fractures or even bone loss.
An important factor in determining a knee replacement’s lifespan is evaluating how the polyethylene insert will hold up over time. Typically, inserts are designed, built and packaged to be resistant to oxidation before being inserted into your knee, which is a prominent cause of premature failure. Exactech’s error was not including a layer of Ethylene Vinyl Alcohol (EVOH) in the packaging, even though their designs implied it would be there. The absence of that layer increases the chances that oxygen can leek into the insert and cause degeneration. Exactech’s ankle inserts suffer from the same packaging defect as their knee inserts. The discovery of this caused the FDA to issue a Class 2 Device Recall. Exactech’s Connexion GXL acetabular liners were recalled because of the the risk of edge-loading, which is when the contact point between the acetabular and femoral components are not aligned, leading to them wearing down. Contrary to Exactech’s polyethylene ankle and knee replacements, these hip liners are also prone to oxidation after they are inserted.
