Exactech Pte Ltd, a corporation that provides healthcare providers with implants and joint replacement technologies, called for some of their products to be taken off the market, as they do not last as long as expected, which can lead to serious problems for some patients. Many of these products have been used in surgeries for 28 years.
The the insert in question, Exactech Ultra-High Molecular Weight Polyethylene (UHMWPE) knee inserts, were said to have an eight-year shelf life. However, complaints from customers who were implanted with them found the the inserts to be faulty and vulnerable to rapid degradation, leading to increased likelihood of additional knee damage and other medical complications.
An important aspect of determining a knee replacement’s lifespan is evaluating how the polyethylene insert will hold up over time. Typically, inserts are designed to be resistant to oxidation before your surgery, which is a prominent cause of premature failure. Exactech designed and manufactured their inserts well, and intended to include a layer of Ethylene Vinyl Alcohol (EVOH) in the packaging to eliminate the risk of oxidation. However, it was discovered that, for whatever reason, EVOH was not included in the packaging, leaving the inserts prone to oxidation and degeneration. The knee replacement failing earlier than expected can cause a range of medical issue for the patient.
Exactech’s polyethylene ankle replacement inserts were recalled because of the same lack of Ethylene Vinyl Alcohol in the packaging, leading to oxidation and premature failure. After several surgeons and ankle replacement recipients complained of the premature failures the Food and Drug Administration to institute a Class 2 Device Recall. If you were given a faulty ankle replacement, it can not only lead to additional surgery or an entirely new ankle replacement, but searing pain, not to mention the extra medical bills and time spent off work.
There are four pieces of a hip replacement:
A pair of studies published in 2020, one titled Early Polyethylene Failure in a Modern Total Hip Prosthesis: A Note of Caution and another called Early failure of a modern moderately cross-linked polyethylene acetabular liner, both warned consumers that the acetabular liner in Extach’s hip replacements often did not have the shelf life they claimed to.
Exactech’s Connexion GXL acetabular liners were recalled because of the the risk of edge-loading, which is when the contact point between the acetabular and femoral components are not aligned, leading to them wearing down. As opposed to Exactech’s polyethylene ankle and knee replacements, these hip liners are also prone to oxidation after the surgery is completed. Before you receive the replacement, the same lack of a second barrier layer as the ankle and knee replacements make the liners more prone to oxidization. After surgery, they can become oxidized in the presence of certain risk factors:
