Exactech Pte Ltd, a limited liability company that manufactures orthopaedic implants, released a global recall in
February 2022 for some of their products, as their shelf-life was found to be shorter than advertised. Many of these
inserts have been implanted in patients for almost three decades.
The the insert in question, Exactech Ultra-High Molecular Weight Polyethylene (UHMWPE) knee inserts, were said to have
an eight-year shelf life. However, reports from patients who were implanted with them found the the inserts to be
defective and susceptible to rapid degeneration, leading to increased chances of further knee damage and other medical
problems.
The lifespan of a knee replacement often comes down to the longevity of their polyethylene insert. Typically, inserts
are designed, manufactured and packaged to be resistant to oxidation before your surgery, which is a major cause of
premature failure. Exactech designed and manufactured their inserts well, and intended to include a layer of Ethylene
Vinyl Alcohol (EVOH) in the packaging to eliminate the risk of oxidation. However, it was discovered that, for whatever
reason, EVOH was not included in the packaging, leaving the inserts prone to oxidation and degeneration. That
degeneration can cause significant harm to the patient.
Exactech’s ankle inserts suffer from the same packaging defect as their knee inserts. This caused the Food and Drug

Exactech’s Connexion GXL acetabular liners were recalled because of the the risk of edge-loading, which is when the
contact point between the acetabular and femoral components are not aligned, leading to them wearing down. The
polyethylene acetabular hip liners are prone to the same oxidation as the knee and ankle inserts both before and after
